Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Johnstown, PA. This full-time position supports planning, organizing, and directing OT services in accordance with regulatory standards to ensure high-quality resident care.

Responsibilities include conducting resident screenings, implementing treatment plans, documenting care, and collaborating with an interdisciplinary team to maximize patient outcomes.

Formación

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure.

Responsabilidades

• Screen residents to determine best intervention per scope of practice.
• Perform therapy and follow short- and long-term goals per care plan.
• Communicate evaluation findings and treatment plans to residents and families.
