Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for our partner in Johnstown, PA, to deliver in-house therapy within a Skilled Nursing Facility.

You will plan, organize, and direct occupational therapy services, screen residents, implement and adjust treatment plans, communicate results to residents and families, coordinate schedules with the care team, and ensure documentation and CMS compliance.

Qualifications

• Associate's degree in Occupational Therapy (OT) required.
• Active OTA licensure in the state(s) of practice required.
• Ongoing continuing education to maintain licensure.

Responsabilités

• Conduct regular screening of residents to determine the best intervention.
• Perform treatment plans aligned with short- and long-term goals.
• Interpret and communicate evaluation findings to residents, families, and the care team.
• Establish individual resident therapy schedules with the team.
• Provide therapy according to plan and ensure proper documentation and billing.
• Carry out other duties to effectively treat patients as assigned.
• Comply with CMS documentation standards daily.
