Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to join an in-house Skilled Nursing Facility in Johnstown, PA. This full-time position supports planning, organizing, and directing occupational therapy services in accordance with federal, state, and local standards.

You will conduct screenings, implement and adjust treatment plans, communicate findings to residents and families, and ensure proper documentation and billing.

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Perform treatment plans and follow short- and long-term goals per plan of care.
• Interpret and communicate evaluation findings to residents, families, and interdisciplinary team.
• Establish individual resident therapy schedules in cooperation with other team members.
• Document and bill occupational therapy services per treatment plan.
• Perform other duties to effectively treat patients as assigned.
• Comply with CMS documentation standards of practice daily with point of care documentation.

Education

Associate's degree in Occupational Therapy

Active OTA licensure in state(s) of practice
