Job summary

Northshore Home Health Care is seeking a Licensed Occupational Therapist to evaluate and treat patients with orthopedic, neurological, developmental, cognitive, and functional needs. You will design care plans, conduct assessments, educate patients and caregivers, and document progress according to policy.

The role includes safe patient transfers and use of modalities like ultrasound and fluidotherapy. Requirements include a Bachelor’s in Occupational Therapy, an active IL license, and BLS

Qualifications

• Bachelor's Degree in Occupational Therapy is required.
• Active license from the Illinois Department of Financial and Professional Regulation.
• Basic Life Support (BLS) certification required.
• Continuing education related to acute stroke care preferred.

Responsibilities

• Evaluate and treat patients with diverse rehabilitative needs.
• Design individualized care plans and document progress.
• Perform patient assessments and educate patients and caregivers.
• Monitor treatment progress and ensure compliance with policies.
• Assist with patient transfers and maintain safety via proper body mechanics.
• Utilize modalities such as ultrasound and fluidotherapy where appropriate.
