Responsibilities

• Conduct comprehensive assessments of individuals with mental illness, behavioral disturbance, substance use issues, or community functioning challenges.
• Assist admission processes (voluntary and involuntary) and coordinate referrals to community resources.
• Coordinate discharge planning with medical staff and participate in performance improvement activities.
• Communicate with outpatient referral sources, families/guardians, and outpatient aftercare providers.
