Qualifications

• Licensed or registered Occupational Therapist per state requirements.
• NBCOT-registered as an OTR where mandated by state.
• Current state OT license/registration to practice.

Responsibilities

• Provide a comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form timely and accurately.
• Develop a comprehensive treatment plan with long- and short-term goals, frequency, duration, and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Deliver treatment to patients using modalities and adjust care while adhering to precautions.

Education

Bachelor/Master/Doctorate in Occupational Therapy recognized by AOTA
