Job summary

Homeside Rehab is seeking a compassionate Occupational Therapist or OTA to join our home health team, delivering therapy to pediatric patients in their homes with schedule flexibility and supportive teamwork.

The role requires NYS OT license, home health experience, and strong clinical assessment with the ability to work independently in a home-based setting.

Qualifications

• Active NYS Occupational Therapy License (OT or COTA) — required.
• Basic computer proficiency for electronic documentation.
• Preferred: 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings.
• Strong clinical assessment and interpersonal communication skills.
• Ability to work independently in a home health environment.
• Valid NYS driver’s license and reliable transportation for home visits.
• Pediatric experience is preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals.
• Provide evidence-based OT interventions to improve motor skills, functional independence, and developmental outcomes.
• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements.
• Collaborate with the clinical team to ensure coordinated and effective treatment.
• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans.

Job Details:

• Job Type: Full-time, Part-time, Per Diem
• Location: Buffalo, NY 14215
• Salary: $65 - $100 per hour
• Schedule Flexibility: Choose your own hours, day and evening shifts available, Monday to Friday, with weekend availability as needed
