Job summary

Aveanna Healthcare is seeking a Licensed Master Social Worker to join our home health care team. You will evaluate, coordinate, and administer social work services to patients and families, guiding care decisions and ensuring compassionate, patient‑centered support in the home.

You will develop care plans with clinical input, connect families to community resources, review progress, and maintain accurate documentation to support compliance and quality outcomes.

Qualifications

• Current state license as required by state law and regulation.
• Master’s degree in Social Work.
• Minimum of one year medical social work experience.

Responsibilities

• Evaluate and coordinate social work services for patients and families.
• Develop and implement plans of care with clinical diagnosis, patient and physician input that include medical and social interventions and measurable goals.
• Coordinate linkages to community medical and health resources and ensure proper implementation of plans of care.
• Maintain accurate and comprehensive client data and ensure timely documentation.
