Job summary

Eventus WholeHealth seeks licensed clinicians to provide recovery-oriented rehabilitation services in skilled nursing and assisted living facilities. You will work within a multidisciplinary team to address mental health needs and improve residents’ quality of life.

Responsibilities include conducting assessments, developing treatment plans, documenting sessions, and coordinating with physicians and facility staff, while maintaining HIPAA compliance and ethical standards.

Qualifications

• Doctoral degree in psychology and a current state license required.
• Master’s degree in social work and a current state license required.
• Experience with assessment and psychotherapy methods.

Responsibilities

• Assess residents with consent to determine needs and follow treatment plans.
• Document treatment plans and sessions per policies; provide records within 48 hours.
• Consult with primary care physicians, families, and facility staff as needed.
• Maintain HIPAA-compliant patient confidentiality and professional standards.
• Collaborate with the health care team to ensure quality care.
