Job summary

Eventus WholeHealth seeks licensed clinicians (LCSW, LISW, LPCC, LCMHC, or LMFT) to provide recovery-oriented rehabilitation in group or individual settings within skilled nursing and assisted living facilities.

You will work as part of a multidisciplinary team, develop treatment plans, document progress, and collaborate with facility staff while upholding HIPAA and ethical standards.

Qualifications

• Doctoral degree in psychology and a current psychologist license or HP license in the practicing state.
• Master’s degree in social work with a current license to practice in the practicing state.

Responsibilities

• Assess residents with consent to determine needs and develop treatment plans.
• Consult with primary care physicians, families, and facility staff as needed.
• Document treatment plans and psychotherapy sessions per procedures; provide timely written reports.
• Meet with care coordinators and staff to review care needs and concerns.
• Maintain open communication with facility staff and adhere to policies, HIPAA and ethics.
• Consult with clinical supervisor for unfamiliar cases.
