Job summary

Eventus WholeHealth is seeking licensed clinicians to join our team providing recovery-oriented rehabilitation in long-term care facilities. You will work with residents in skilled nursing and assisted living settings, delivering group or individual therapy and coordinating with a multidisciplinary team.

The role emphasizes treatment planning, collaboration with physicians, and maintaining patient confidentiality while improving quality of life for residents.

Qualifications

• Doctoral degree in psychology with current license as a Psychologist and Health Services Provider.
• Master's degree in social work and current state license for therapists (LCSW/LISW/LPCC/LCMHC/LMFT).

Responsibilities

• Assess residents with consent to determine needs and develop treatment plans.
• Consult with physicians, families, facility staff, and Eventus support staff as needed.
• Document treatment plans and psychotherapy sessions per policies and within required timelines.
• Coordinate with facility staff on care needs and communicate findings.
• Maintain HIPAA compliance and professional ethics at all times.
