Job summary

Eventus WholeHealth delivers an integrated care model to adults in skilled nursing and assisted living facilities. We seek licensed clinicians (LCSW, LISW, LPCC, LCMHC, LMFT or licensed psychologist) to provide recovery-oriented rehabilitation in group or individual settings.

You will work as part of a multidisciplinary team in long‑term care to improve residents' quality of life. Responsibilities include assessing residents, coordinating with care teams, documenting plans, and maintaining HIPAA

Qualifications

• Doctoral degree in psychology and an active psychology license.
• MSW and an active state license to practice social work.
• Knowledge of DSM and psychotherapy techniques.
• Excellent written and verbal communication; ability to collaborate with team.

Responsibilities

• Assess each resident with consent and develop treatment plans.
• Consult with primary care physicians, family, facility staff, and support staff.
• Document treatment plans and psychotherapy sessions per policy.
• Meet with facility staff to review current care needs and communicate findings.
• Maintain confidentiality and HIPAA compliance; adhere to ethics and regulations.
