Interim Healthcare of Scranton is seeking a Licensed Medical Social Worker (LMSW) to perform per diem home health visits in the Scranton area. You will conduct assessments, provide counseling, and connect patients with community resources while working with an interdisciplinary team.

The role offers flexible scheduling, meaningful patient impact, and a pay rate of $85 per visit. Recent graduates or experienced LMSWs are welcome to apply, with PA licensure required.

Qualifications

• Current MSW license in Pennsylvania.
• Experience in home health, hospice, or similar settings preferred.
• Knowledge of local community resources in Lackawanna and surrounding counties.
• Strong communication skills and ability to work independently and in a team.
• Reliable transportation and valid driver's license.

Responsibilities

Key Responsibilities for the Licensed Medical Social Worker LMSW:

• Conduct social work assessments and develop personalized care plans for home health patients.

• Provide counseling and support to patients and families to help them adjust to illness, injury, or the challenges of aging.

• Assist patients in connecting with community resources, services, and support networks.

• Collaborate closely with the interdisciplinary healthcare team, including nurses, therapists, and physicians, to ensure holistic patient care.

• Offer crisis intervention, advocacy, and guidance to patients and their families as needed.

• Maintain thorough and timely documentation of services provided in compliance with healthcare regulations.

Minimum Qualifications for the Licensed Medical Social Worker LMSW:

• Required: CurrentMSW license in the state of Pennsylvania.

• Prior experience in home health, hospice, or a similar healthcare setting preferred.

• Knowledge of local community resources and support services in Lackawanna, Wyoming, Wayne, and Susquehanna counties.

• Strong communication skills and the ability to work both independently and within a team.

• Reliable transportation and a valid driver's license.

• CPR and First Aid certification preferred but not required.
