Job summary

Interim Healthcare of Scranton is seeking a Licensed Medical Social Worker (LMSW) to perform per diem home health visits in the Scranton area. You will conduct assessments, provide counseling, and connect patients with community resources while working with an interdisciplinary team.

The role offers flexible scheduling, meaningful patient impact, and a pay rate of $85 per visit. Recent graduates or experienced LMSWs are welcome to apply, with PA licensure required.

Qualifications

• Current MSW license in Pennsylvania.
• Experience in home health, hospice, or similar settings preferred.
• Knowledge of local community resources in Lackawanna and surrounding counties.
• Strong communication skills and ability to work independently and in a team.
• Reliable transportation and valid driver's license.

Responsibilities

• Conduct social work assessments and develop care plans for home health patients.
• Provide counseling and support to patients and families to adapt to illness and aging.
• Help connect patients with community resources and services.
• Collaborate with nurses, therapists, and physicians for holistic care.
• Offer crisis intervention and advocacy as needed.
• Document services thoroughly and timely in accordance with regulations.
