Job summary

Interim Healthcare of Scranton is seeking a Licensed Medical Social Worker (LMSW) to perform per diem home health visits. Earn $85 per visit with a flexible schedule, allowing you to set your own hours while making a meaningful impact in the local community.

Responsibilities include social work assessments, care planning, counseling, and coordinating with an interdisciplinary team. Pennsylvania LMSW license required; CPR/First Aid preferred but not mandatory.

Qualifications

• Current MSW license in Pennsylvania.
• Experience in home health, hospice, or similar.
• Knowledge of local resources in Lackawanna, Wyoming, Wayne, and Susquehanna counties.
• Strong communication skills and ability to work independently and in a team.
• Reliable transportation and a valid driver’s license.
• CPR and First Aid certification preferred but not required.

Responsibilities

• Conduct social work assessments and develop personalized care plans for home health patients.
• Provide counseling and support to patients and families to help them adjust to illness, injury, or aging.
• Assist patients in connecting with community resources and support networks.
• Collaborate with the interdisciplinary healthcare team to ensure holistic patient care.
• Offer crisis intervention, advocacy, and guidance to patients and families as needed.
• Maintain thorough documentation of services in compliance with healthcare regulations.
