The LMSW will perform psychosocial assessments, counseling, and referrals to support patients and families in home health and hospice settings. The role requires MSW degree and state licensure (LMSW/LCSW/LICSW), CPR certification, and reliable transportation.

Qualifications

• Master's or Doctoral Degree from an accredited school of Social Work.
• Active LMSW/LCSW/LICSW licensure in the state of practice.
• Minimum of one year MSW experience in a healthcare setting.
• Working knowledge of community resources and strong interpersonal skills.
• CPR certification current.
• Reliable transportation with a valid driver's license and auto liability insurance.

Responsibilities

• Conduct ongoing psychosocial assessments for home health and hospice patients.
• Develop and evaluate the care plan with patient, representative, and caregiver.
• Provide counseling and education as ordered by the physician.
• Maintain accurate documentation of assessments and interventions.

Job Description

First Choice Home Health & Hospice / Jet Health Inc. is currently seeking a full-time, Licensed Medical Social Worker (LMSW) to join our team to serve our patients currently enrolled in our home health and hospice services in the Boise, ID region and surrounding areas. The successful candidate will provide coordinated care in the home to patients of all age groups, performing psychosocial assessments, analysis, counseling, and referrals to meet the needs of the patient, client, and family. This position offers an opportunity to make a significant impact on the lives of patients and their families in a supportive and dynamic environment.

Responsibilities

• Conduct ongoing clinical assessments of home health and hospice patients to identify psychosocial, financial, environmental, and community resource needs.
• Develop and evaluate the plan of care in collaboration with the patient, client, representative (if any), and caregiver(s).
• Provide counseling and education to patients, clients, and caregivers as ordered by the physician and indicated in the plan of care.
• Maintain accurate and timely documentation of clinical assessments, interventions, and communication with other healthcare practitioners.
• Utilize effective interpersonal relations and communication skills to facilitate positive outcomes for patients and their families.
• Collaborate with the interdisciplinary team to promote coordination of patient care and ensure comprehensive support.
• Participate in the Agency's Quality Assessment and Performance Improvement (QAPI) program and in-service training.
• Stay updated on public and private eligibility standards and requirements, maintaining professional growth and competency.
• Promote the Agency's philosophy and administrative policies to ensure the delivery of quality care.

Requirements
