Job summary

Klarus Home Health / Jet Health Inc. seeks a Licensed Medical Social Worker (LMSW) to join our team serving patients in the Albuquerque, NM region and surrounding areas. You will perform psychosocial assessments, counseling, and referrals to meet patient and family needs in a home health setting.

The role offers the opportunity to make a significant impact in a dynamic, supportive environment. Requires MSW and active LMSW/LICSW/LCSW licensure, CPR, and 1 year healthcare MSW experience.

Qualifications

• Master's or Doctoral Degree from an accredited school of Social Work.
• Active social work licensure (LMSW, LICSW, or LCSW) in the state of practice.
• Minimum of one year experience as an MSW in healthcare.
• Working knowledge of community resources and good interpersonal skills.

Responsibilities

• Conduct ongoing clinical assessments of home health patients to identify psychosocial, financial, environmental, and community resource needs.
• Develop and evaluate the plan of care in collaboration with the patient, client, representative (if any), and caregiver(s).
• Provide counseling and education to patients, clients, and caregivers as ordered by the physician and indicated in the plan of care.
• Maintain accurate and timely documentation of clinical assessments, interventions, and communication with other healthcare practitioners.
• Collaborate with the interdisciplinary team to promote coordination of patient care and ensure comprehensive support.
• Participate in the Agency's QAPI program and in-service training.

Requirements

Salary Description

$55 per visit
