Job summary

UnitedHealth Group is seeking a Medical Social Worker to deliver psychosocial support, assessment, and care coordination for patients and families in the home setting. You will work closely with an interdisciplinary team to connect patients with community resources and provide counseling as part of comprehensive care transitions.

The role requires a Master’s or Doctorate in Social Work, LMSW/LCSW licensure, CPR certification, and a minimum of one year in a health care setting.

Qualifications

• Master’s and/or Doctorate degree from a school of social work, accredited by the Council on Social Work Education.
• Active licensure in Medical Social Work (LMSW) or Clinical Social Work (LCSW).
• Driver's License and access to reliable transportation.
• Current CPR certification.
• Has one year of social work experience in a health care setting.

Responsibilities

• Provide psychosocial support, assessment, and care coordination to patients and families in the home.
• Collaborate with the interdisciplinary team to connect patients with community resources and counseling.
• Educate patients and family members on social work interventions and treatment plans.
• Performs care planning, transfer and discharge functions.
• Identify risks to patient health and communicate with the care team to coordinate care.
