We are seeking a dedicated and passionate Licensed Social Worker (LMSW - $66+/hour!) to join our team for hospital contract positions around NYC. These are full-time, M-F/9a-5p roles with contracts available in Manhattan, Brooklyn, Bronx, and Queens.

Hospital LMSW Social Worker Duties Include:

• Reviewing admissions information to identify patient needs, such as special diets that align with personal or religious beliefs
• Serving as an advocate for patients and advising medical care team members about patients’ needs
• Conducting discharge counseling to ensure patients have proper support, accommodations, supplies, and equipment upon returning home
• Referring patients to government agencies and nonprofit organizations for assistance with homelessness, food insecurity, unemployment, or transportation issues
• Coordinating care between medical providers for complex cases

Hospital LMSW Social Worker Requirements:

• Master's Degree in Social Work
• LMSW required
• 1-2 years of prior medical social work experience highly preferred
