Job summary

HCA Healthcare Live Oak Mental Health and Wellness is seeking a Licensed Lead Intake Clinician to join our Charleston-area team. You will lead intake assessments, coordinate multidisciplinary care, and guide discharge planning for adults and youth with complex psychiatric needs.

The role emphasizes culturally competent care, safety, and collaboration with physicians, nurses, social workers, and other professionals to ensure smooth transitions and high-quality treatment in a fast-paced inpatient

Qualifications

• As a licensed clinician, demonstrate safe, ethical practice and timely documentation.
• Experience coordinating care across multidisciplinary teams and settings.
• Ability to assess risk factors and modify treatment plans accordingly.

Job Summary And Qualifications

• Demonstrates advanced clinical skills and ability to assess patients to determine appropriate level of care.
• Identifies cultural considerations, gender identity and preferred language for patients and family
• Demonstrates advance practice in assessments, and all other documentation timely and in compliance with requirements (as appropriate to scope)
• Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition
• Identifies risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement)
• Recognizes and acts upon unexpected assessment findings
• Leads in ensuring a safe structured milieu with evidence based treatment modalities/resources
• Maintains active and timely communication with patients, families, multidisciplinary team and continuum of care providers
• Provides education and guidance regarding state commitment processes, abuse reporting requirements, duty to warn and guardianship processes.
• Assesses, collaborates, coordinates and provides guidance on complex discharge planning needs and readiness for discharge with the multidisciplinary team.
• Develops and maintains a community resource guide including access to public assistance/alternatives.
• Facilitates and participates in Community Education, Activities and Support Groups (as appropriate to scope)
• Demonstrates appropriate application of age/developmental stage specific standards, policies and procedures, and guidelines in assessing and treating patients to ensure patient safety.
• Maintains professionalism and uses good communication skills in interacting with peers, staff, patients, families and in resolving conflicts.
• Demonstrates understanding of and ability to maintain clear professional boundaries with peers, patients and families.
• Serves as an educational resource to patients, families, interdisciplinary team members, payer representatives, and other parties as appropriate regarding behavioral services
• Reviews, coaches and assists with training of new staff/interns as assigned within approved practice and program guidelines.

What qualifications you will need:

• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
• (LCSW) Clinical Social Worker, or (LMFT) Licensed Marriage and Family Therapist, or (LPC/LMHC) Licensed Professional/Mental Health Counselor

Live Oak Mental Health and Wellness Center

We give inpatient stabilization treatment to patients who seek treatment for acute psychiatric symptoms which cannot be addressed on an outpatient basis. Common disorders of patients treated in this program include but are not limited to: major depressive disorder, schizoaffective disorder, schizophrenia, bipolar disorder, adjustment disorders, post-traumatic stress disorder, impulse control disorder and a variety of co-morbid personality disorders. Our patient population is both adult and adolescent. We offer 24 hour availability of a full range of services. We have the capability to institute rapid lifesaving interventions. Our physician availability is 24 hours a day to institute needed modification to the treatment plan. Psychiatrist involved in the direction and management of the treatment program. Round the clock skilled nursing care implementing the treatment plan and providing monitoring and assessment of the patient's condition and response to treatment.
