Job summary

HCA Healthcare Live Oak Mental Health and Wellness is seeking a Licensed Outpatient Therapist to provide supervised mental health services across individual, family, and group settings. You will develop treatment plans, deliver psychoeducation, and lead therapy sessions using evidence-based approaches while coordinating care with a multidisciplinary team.

Requirements include a Master's degree and licensure such as LMSW, LCSW-C, LPC-A, LPC, LMFT, LCSW or LMHC.

Qualifications

• Master's degree in social work or counseling required.
• Licensure in one or more of LMSW, LCSW-C, LPC-A, LPC, LMFT, LCSW, LMHC required.
• Basic Cardiac Life Support and Nonviolent Crisis Intervention within 30 days of start date.

Responsibilities

• Conduct one-on-one, group, and family psychoeducational and process groups.
• Develop treatment plans and attend planning meetings.
• Assess environment and safety; report changes in patient condition.
• Identify risk factors and interventions for patients with precautions.
• Complete assessments, group notes, and documentation per requirements.
• Collaborate with multidisciplinary team for discharge planning needs.
• Conduct post-discharge follow-up as indicated.

Job Summary And Qualifications

The Mental Health Professional II is to deliver exceptional, patient-focused care by providing supervised mental health services to patients and their loved ones. Their responsibilities include conducting assessments, offering psycho-education, leading process groups, developing treatment plans, and facilitating therapeutic counseling sessions using evidence-based interventions in one-on-one, family, and group therapy.

What you will do in this Role

• Conducts one-on-one, group, and family psychoeducational and process groups.
• Develops treatment plans and attends treatment planning meetings and reviews on an ongoing basis.
• Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition.
• Identifies risk factors, screening criteria, and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).
• Completes assessments, group notes, and all other documentation promptly and in compliance with requirements, as appropriate to scope.
• Collaborates with the multidisciplinary team to assess and coordinate patients’ discharge planning needs and readiness for discharge.
• Conducts post-discharge follow-up as indicated.

What qualifications you will need

• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
• Masters Degree - REQUIRED

Live Oak Mental Health and Wellness Center

is a part of Trident Health. We give inpatient stabilization treatment to patients who seek treatment for acute psychiatric symptoms which cannot be addressed on an outpatient basis. Common disorders of patients treated in this program include but are not limited to: major depressive disorder, schizoaffective disorder, schizophrenia, bipolar disorder, adjustment disorders, post-traumatic stress disorder, impulse control disorder and a variety of co-morbid personality disorders. Our patient population is both adult and adolescent. We offer 24 hour availability of a full range of services. We have the capability to institute rapid lifesaving interventions. Our physician availability is 24 hours a day to institute needed modification to the treatment plan. Psychiatrist involved in the direction and management of the treatment program. Round the clock skilled nursing care implementing the treatment plan and providing monitoring and assessment of the patient's condition and response to treatment.
