Job summary

Health Connect America is seeking a licensed Family Centered Treatment Therapist to provide evidence-informed services to children, families, and youths in home or community settings. The role emphasizes family engagement, care coordination, and multidisciplinary collaboration.

The position requires a Master's degree and Ohio licensure eligibility, with optional FCT certification preferred. A comprehensive benefits package, flexible scheduling, and ongoing professional development are provided.

Qualifications

• Minimum of Master's Degree in Counseling, Social Work, or related field is required.
• Two years' experience working with youth or families.
• Current Family Centered Treatment Certification is preferred.

Responsibilities

• Provide Family Centered Treatment (FCT) Clinical Services to children, adolescents, and families in home, school, or community settings.
• Complete FCT Checkoffs and training within designated deadlines.
• Deliver FCT services with a family focus and maintain documentation in the Electronic Health Record.
• Provide 24/7 on-call coverage to a caseload of families and children.
• Develop Service Plans in coordination with Treatment Team and communicate with team members.
• Coordinate discharge planning and aftercare needs with community resources.
