Job summary

Health Connect America is hiring a licensed Family Centered Treatment Therapist to provide clinical FCT services for children, adolescents, and families in home, community, and clinical settings. You will complete required trainings, develop and monitor Service Plans, and collaborate with a Treatment Team to ensure coordinated care.

Qualifications

• Master's degree in Counseling, Social Work, or related field is required.
• Two years' experience with youth or families.
• Current Family Centered Treatment Certification is preferred.

Responsibilities

• Provide Family Centered Treatment (FCT) clinical services to children, adolescents, and families in home, community, and office settings.
• Complete FCT checkoffs and internal trainings within deadlines.
• Develop Service Plans with Treatment Team and coordinate with multiple providers.
• Deliver psychoeducation and intervene with families to move through four phases of treatment.
• Coordinate discharge planning and access community resources for aftercare.

Education

Master's degree required in Counseling, Social Work or related field

Youth or family work experience (2 years)

Ohio licensure eligible (LISW/LKW? Licensure names: LSW, LISW, LPC, LMFT, or LPCC)
