Job summary

BayMark Health Services, Inc. seeks a Full Time Licensed Counselor in Lake Charles, LA to support patients in Medication Assisted Treatment settings. Responsibilities include individual and group counseling, assessment, and collaborative care planning with a multidisciplinary team.

The role requires current Louisiana licensure (LCSW, LPC, LMFT, LAC, LRC) and prior experience with chemically dependent individuals; STAR LRP may provide loan repayment options for eligible locations.

Qualifications

Responsibilities

• Provide individual and group counseling services; direct service at regularly scheduled intervals; coordinate services throughout treatment.
• Conduct initial assessment and evaluation to determine eligibility for services.
• Complete Bio-psychosocial assessment.
• Establish individualized treatment plan with each patient and review as clinically indicated.
• Respond to patient grievances and complaints.
• Develop aftercare and discharge plans.
• Provide support to 4 other locations/clinics.
• Other related duties as determined by supervisor.

Full Time-Licensed Counselor

The licensed counselor willbe responsible foroffering support andassistance to patientsemployed in Medication Assisted Treatment for opioid use disorder. Utilizing evidence-based practices and a trauma-informed approach, the counselor will help patients develop individualized coping techniques and symptom management skills that enablethen to experience improved quality of life. Through collaboration with doctors, nurses and a multidisciplinary team, the counselor will play a vital role in evaluating patient progress and developing an individualized plan of care.

Essential Job Functions:

• Provide individual and group counseling services; direct service at regularly scheduled intervals based on patient need, best practices and state regulations; coordinate all services throughout treatment.
• Conducts initial assessment and evaluation to determine eligibility for services.
• Complete Bio-psychosocial assessment.
• Establish individualized treatment plan with each patient and review as clinically indicated.
• Respond, as needed, to patient grievances and complaints.
• Develop aftercare and discharge plans.
• Will provide support to 4 other locations/clinics.

Qualifications:

• Current Louisiana licensure - LCSW, LPC, LMFT, LAC, LRC
• Previous experience working with chemically dependent individuals.
• Must demonstrate a positive, non-judgmental, and accepting attitude toward our patient population
• Ability to collaborate with compassionate and knowledgeable counseling team members
• Satisfactory criminal background check and drug screen
