We are seeking a compassionate, proactive Community Health Care Worker (CHW) to support our pharmacy’s efforts in improving patient care and health outcomes. This role will serve as a vital link between the pharmacy, patients, and the broader community. You will work both onsite at the pharmacy, remotely, and within the surrounding neighborhoods to provide health education, coordinate care, and assist with billing processes.

Key Responsibilities:

Serve as a liaison between patients, pharmacy staff, and healthcare providers.

Provide culturally appropriate health education and outreach to improve patient understanding and engagement.

Assist patients with accessing health and social services, including transportation, insurance, and wellness resources.

Conduct follow-ups and wellness checks via phone, video, or in-person visits.

Educate patients about medication adherence and chronic disease management.

Support billing processes, including insurance verification, claims assistance, and basic administrative tasks.

Track and report patient interactions and outcomes using electronic health records or designated systems.

Participate in community health fairs, vaccination drives, and wellness events.

Maintain patient confidentiality and comply with all regulatory and ethical standards.

Qualifications:

Certification as a Community Health Worker (CHW) required.

Prior experience in a pharmacy, clinic, or health outreach role preferred.

Basic knowledge of healthcare billing, insurance claims, and patient assistance programs.

Strong interpersonal, organizational, and communication skills.

Ability to work independently and in diverse environments (in-store, remote, community).

Spanish-speaking candidates strongly preferred.

Proficient in using digital tools for documentation and communication (e.g., EHR systems, telehealth platforms).
