Job description

LOCATION: Remote - must live in or near McDowell or Rutherford County, North Carolina. The person in this position is required to maintain residency in North Carolina or within 40 miles of the NC border. This position requires travel.

GENERAL STATEMENT OF JOB

The Innovations Care Manager (Innovations CM) is responsible for providing proactive intervention and care coordination to eligible Vaya Health members to ensure appropriate assessment and services. The Innovations CM works with members and care teams to address care gaps across various health domains, support transition planning, and track discharge follow-ups to prevent hospitalization. This mobile position involves work in members' communities and collaboration with staff, relatives, caregivers, providers, and community stakeholders. Essential functions include:

  1. Using Vaya's Care Management software platform and maintaining accurate documentation.
  2. Outreach, engagement, and compliance with HIPAA regulations.
  3. Performing comprehensive health risk assessments (HRA) covering social, mental, physical, and social needs.
  4. Supporting care planning, interdisciplinary team participation, transitional and crisis care management, and diversion from institutional placements.

Residency Requirements: Must reside in or near the counties served to effectively deliver in-person contacts.

ESSENTIAL JOB FUNCTIONS

Assessment, Care Planning, and Team Collaboration:

  • Identify, assess, and develop person-centered care plans addressing mental health, substance use, medical, and social needs.
  • Link members to appropriate services, conduct HRAs, and support care plan development.
  • Review assessments, support treatment goal formulation, and involve members and care teams.
  • Update care plans annually or with significant life changes.

Support, Monitoring, Documentation, and Compliance:

  • Participate in multidisciplinary meetings, monitor service delivery, and ensure compliance with standards and regulations.
  • Coordinate Medicaid eligibility, create care management documentation, and ensure timely submission of service requests.

Other Duties:

  • Perform additional responsibilities as assigned.

KNOWLEDGE, SKILLS, & ABILITIES

  • Effective communication, relationship-building, organizational skills, and decision-making abilities.
  • Proficiency in Microsoft Office and care management systems.
  • Understanding of mental health, substance use, I/DD, TBI services, and NC Medicaid programs.

EDUCATION & EXPERIENCE

Bachelor's degree in a relevant field required, with specific experience working with I/DD, TBI, or LTSS populations. Additional experience requirements depend on educational background, including licensure if applicable.

Licensure/Certification: RN licensure by the North Carolina Board of Nursing if applicable.

PHYSICAL REQUIREMENTS

  • Visual acuity, mobility, ability to drive, and mental concentration required.

RESIDENCY REQUIREMENT: Must reside in North Carolina or within 40 miles of the border.

SALARY: Based on qualifications and experience; eligible for overtime.
