Job summary

Integrated Resources, Inc. seeks a licensed care coordinator to perform in-depth health risk assessments and develop care plans with the interdisciplinary team. You will coordinate services, address gaps in care, and support members and families in accessing appropriate resources.

The role requires DC licensure (LCSW-C/LCPC/LCMFT) with 5+ years post-master experience. Training is three weeks in-office, then remote work; the client will pay for the license. Annual salary is $70K.

Qualifications

• Licensed as LCSW-C or LCPC or LCMFT with 5+ years post-master experience.
• DC licensed candidate required; DC license to be obtained within 30 days; client pays for license.

Responsibilities

• Conduct in-depth health risk assessments and comprehensive needs assessments.
• Develop and communicate treatment plans for authorization of services and transition to home care.
• Coordinate and monitor care plans to improve health and quality of life outcomes.
• Identify gaps in care and advocate for members’ needs with the interdisciplinary team.
• Educate providers, staff, members and families on care coordination and cost-effective delivery of services.
• Collaborate with the Interdisciplinary Care Plan team to ensure appropriate interventions across the continuum.
