Your Role as a Home Health Medical Social Worker (MSW)

As a Home Health Medical Social Worker (MSW), you will provide psychosocial support and social work services to patients receiving care in their homes. You will play an important role in identifying social, emotional, financial, and environmental factors that may impact patient outcomes and overall well-being.

Responsibilities Include:

• Conduct psychosocial assessments for patients and families
• Assist physicians and interdisciplinary team members in understanding social and emotional factors impacting health conditions
• Develop and coordinate care plans addressing psychosocial needs
• Connect patients and families with community resources and support services
• Provide counseling, education, and emotional support to patients and caregivers
• Assist with discharge planning and care transitions
• Collaborate with physicians and interdisciplinary care teams to improve patient outcomes
• Maintain timely and accurate documentation in accordance with Medicare guidelines and agency policies
• Complete documentation efficiently using AI-assisted EMR tools

Qualifications
