Job summary

Community Care of West Virginia Inc in Clay, WV seeks an LICSW to provide psychosocial assessments, develop treatment plans, and deliver mental health and substance abuse interventions. The role includes care planning with patients and families, resource linkage, and collaboration with the care team to meet treatment goals.

You will assess patient education needs, support self-management, and may supervise LGSW/LCSW staff.

Qualifications

• Master’s degree from a CSWE-accredited program.
• Licensed by the State Board of Social Work.
• Experience with Medicare/Medicaid populations and telephonic engagement.
• Two years of supervised social work experience in healthcare setting.
• Certified Clinical Alcohol, Tobacco, and Other Drug Social Worker (preferred).

Responsibilities

• Engage patients and caregivers in active care planning addressing behavioral, clinical, social, and environmental needs.
• Create and maintain inventory of local community resources and connect patients with services.
• Assess patient knowledge of their medical condition and provide education and self-management support.
• Provide direct care management to high-risk patients and clinical consultation to the care team.
• Deliver psychosocial assessments, diagnoses, and treatment recommendations.
• Provide in-office mental health and substance abuse interventions as requested.
• Support collaborative documentation including goals, progress notes, and billing.
• Promote inter-department cooperation and participate in committees as needed.
• Perform additional duties to ensure seamless patient care and mission alignment.
