This role is critical in addressing social drivers of health, developing individualized care plans, and contributing to effective discharge planning and crisis intervention. The LCSW also plays an integral part in program development, quality improvement initiatives, and mentoring of social work team members. The LCSW collaborates closely with physicians, nurses, case managers, and allied health professionals to ensure a holistic, patient-centered approach to care.
