Job summary

UW Health is seeking a Licensed Clinical Social Worker (LCSW) at our northern Illinois facility. The role operates 100% FTE, Monday–Friday 8a-5p with occasional later hours; clinic location at 2601 N Main St, Rockford, IL. Qualifications include MSW, LCSW/LCPC, and at least two years of clinical mental health experience. Strong teamwork and patient-focused care are essential in this setting.

Qualifications

• Master's Degree in Social Work (MSW) required.
• LCSW license or LCPC; Illinois license required.
• Minimum 2 years of clinical mental health experience; individual and/or group casework preferred.
• Valid State Driver's License and proof of liability insurance.
• BLS/CPR certification required.

Responsibilities

• Provide clinical social work services to patients in mental health settings.
• Collaborate with care teams to develop and implement patient care plans.

Work Schedule:

100% FTE, Monday - Friday 8a-5p; scheduled hours may very within this timeframe; one day until 6p, Fridays off at 4p. You will work at UW Health N. Main St. Clinic 2601 N Main St.

Additional components of compensation may include:

• Evening, night, and weekend shift differential
• Overtime

• At least two years experience in individual and/or group casework. Preferred

Licenses & Certifications
