Job summary

The Licensed Clinical Social Worker (LCSW) at Community Health Centers, Inc. assists patients and families in understanding diagnoses, emotions, and making treatment decisions.

You will work with doctors, nurses, and allied health professionals using case management to address social, financial, and psychological needs related to health conditions. Requirements include an MSW from a CSWE-accredited program, an LCSW license, and at least 2 years in a healthcare setting (ambulatory preferred).

Qualifications

• Bachelor's degree in healthcare or related field is required.
• Master's degree in Social Work from CSWE-accredited school.
• LCSW license is required.
• Current BLS certification or must be obtained within 30 days.
• Must obtain FARS and CFARS within first 2 weeks.
• 2 years healthcare experience; ambulatory setting preferred.
• Must be familiar with Hispanic culture and multicultural settings.
• Willing to enroll in language and multicultural coursework if necessary.
• Supervisory experience preferred.

Responsibilities

• Assist patients and families in understanding illness, emotions, and counseling decisions.
• Collaborate with clinical staff using case management to address social, financial and psychological issues.
• Support coordination of care across the CHC center.
