Job summary

The Licensed Clinical Social Worker at Community Health Centers supports patients and families by addressing social, financial, and psychological aspects of illness, while guiding decisions with counseling and case management. Qualifications

• Bachelor's degree in healthcare or related field.
• Master's Degree in Social Work from a CSWE-accredited school.
• LCSW certification required.
• BLS certification within 30 days of employment.
• FARS and CFARS certifications within first 2 weeks.
• Minimum 2 years healthcare experience; ambulatory center preferred.
• Familiarity with multicultural settings and willingness to pursue language coursework.

Responsibilities

• Assist patients and families in understanding illness and coping with a diagnosis.
• Provide counseling about decisions and treatment options.
• Collaborate with CHC clinicians using case management to address social, financial and psychological needs.
• Maintain confidentiality and comply with privacy standards.
