You will provide individual and group therapy, complete psychosocial assessments, and collaborate with physicians and care teams to deliver holistic, patient-centered care.

Qualifications

• Active Florida LCSW license required.
• Master's degree in Social Work (MSW) from an accredited program.
• Experience in a clinical, medical, or healthcare setting preferred.
• Strong knowledge of psychosocial assessment, crisis intervention, and evidence-based therapy.
• Excellent communication and interpersonal skills with a patient-centered approach.
• Ability to work collaboratively within a multidisciplinary medical team.
• Proficiency with electronic health records (EHR) systems.

Responsibilities

• Provide individual and group therapy to patients managing chronic endocrine disorders such as diabetes, thyroid disease, and hormonal conditions.
• Conduct psychosocial assessments and develop individualized treatment plans.
• Collaborate with physicians, nurses, and care teams to deliver integrated, whole-patient care.
• Offer crisis intervention, counseling, and mental health support to patients and families.
• Connect patients with community resources, support groups, and social services.
• Maintain accurate clinical documentation in compliance with HIPAA and regulatory standards.
• Participate in care coordination meetings and multidisciplinary team discussions.
