Central Maine Medical Center in Lewiston is the flagship facility of Central Maine Healthcare. CMMC is a 250-bed, not-for-profit, Level III Trauma Center, offering comprehensive inpatient and outpatient services to the people of central Maine. CMMC provides 24-hour emergency care, and medically supervised orthopedic rehabilitation programs. Our rehabilitation facilities are part of our all-in-one, single floor orthopedics unit designed to support the initial states of recovery, helping patients get back home and get moving again sooner. Central Maine Healthcare covers all of patients physical and mental health needs.
The Case Manager is responsible for coordinating social services and discharge planning to support patients throughout their continuum of care. This role assesses patient and family needs, identifies and arranges appropriate community and healthcare resources, and collaborates closely with providers, nursing, and interdisciplinary teams to ensure safe, effective care transitions. The Case Manager supports inpatient, outpatient, and Emergency Department populations while promoting compliance with regulatory standards, patient safety initiatives, and performance improvement efforts. This position requires strong critical thinking, sound judgment, and the ability to work independently in a fast-paced healthcare environment. Bachelor's degree required, Master's degree preferred, with a current Kentucky Social Work license and a minimum of three years of related social work experience.
