Job summary

WellStar is seeking a Behavioral Health Assessor-Licensed to perform comprehensive assessments in ED and acute care settings, including tele-mental health. The role collaborates with physicians and care teams to plan safe transitions and provides psycho-education and interventions for patients with behavioral health needs.

Responsibilities include suicide risk assessment, discharge planning, and documentation in the EMR. Requires licensure or eligibility and 3+ years of relevant experience.

Qualifications

• Master's Degree MSW from an accredited school or Master's in counseling from CACREP program.
• Licenses: BLS certification; GA LCSW or LPC or similar preferred.
• Experience: minimum 3 years with adult/child behavioral health.

Responsibilities

• Perform comprehensive face-to-face and tele-mental health assessments in ED or acute care.
• Collaborate with physicians, social workers and care team to plan safe care transitions.
• Provide suicide risk assessments and discharge planning.
• Document assessments and care plans in EMR with clinical notes.
• Develop safety plans and coordinate referrals to appropriate levels of care.
