Wellstar Health System is seeking a Licensed Behavioral Health Assessor to perform comprehensive assessments, including face-to-face and tele-mental health services, for patients with behavioral health and substance-related needs. You will collaborate with physicians and the care team to determine the appropriate level of care and discharge planning.

Responsibilities include suicide risk assessments, safety planning, documentation in the EMR, and coordination of referrals and legal documentation

Qualifications

• Master's degree in social work or counseling from CACREP accredited program.
• GA licensure as LCSW/LMSW or LPC/LPPC preferred.
• Minimum 3 years experience with adult/child behavioral health.

Responsabilités

• Conduct comprehensive in-person or tele-mental health assessments.
• Provide suicide risk assessments.
• Collaborate with physicians, social workers, and care team to plan care.
• Coordinate discharge planning and safety planning with patients and families.
• Document assessments, plans, and contacts in the EMR.
• Assist with referrals and legal documentation as needed.
