Fello is seeking a Licensed Practical Nurse (LPN) to provide person-centered nursing care in a group home setting, under the supervision of the Nurse Case Manager/Delegating Nurse. You will work with the Regional Manager and DSPs to support independence and community inclusion for people served.

The ideal candidate has a Maryland LPN license, at least one year of experience with people with disabilities, and a valid Maryland driver’s license with two points or less.

• Valid Maryland driver’s license with no more than two points.
• Must complete all required Fello and Developmental Disabilities Administration trainings.

• Coordinate medical appointments and immunizations and maintain health documentation in iCM and other EMR systems.
• Support Activities of Daily Living (ADLs) to promote independence and community inclusion.
• Communicate with persons served, families, coworkers, and stakeholders in a timely, professional manner.

The LPN works with the Regional Manager and Direct Support Professionals (DSPs) to promote independence and community inclusion for people served. Responsibilities include delivering care aligned with each person's Nursing Care Plan and Individual Plan, supporting Activities of Daily Living (ADLs), coordinating medical appointments and immunizations, and maintaining health documentation in iCM and other electronic systems. The LPN must meet DSP qualifications and complete all required training. This is an essential position, and staff may not leave their shift until appropriate coverage has arrived.The ideal candidate is self-motivated, creative, and innovative in providing community integration and support services. They are a team player who responds to change or crisis in a calm and productive manner and who can respectfully support the varied needs of persons served. They build self-esteem and confidence through positive interactions and trusting relationships. They communicate in a timely, professional manner with persons served, their families, coworkers, and other stakeholders. They also have knowledge of databases and electronic medical records and are committed to learning about health services for people with disabilities.This position requires a current Maryland LPN license issued by the Maryland Board of Nursing and at least one year of experience providing nursing services for people with disabilities. Candidates must have a valid Maryland driver's license with no more than two points. They must also be able to complete all required Fello and Developmental Disabilities Administration (DDA) trainings.This position is part-time or full-time, based in an assigned group home, with a work schedule that varies based on the needs of persons served. Schedules are flexible and include on-call hours. This position involves limited travel, including regular trips to the Severn campus for meetings and trainings.
