Job summary

Fello Severn, MD is seeking a licensed practical nurse (LPN) to provide person-centered nursing care in a group home setting. You will work under the guidance of the Nurse Case Manager/Delegating Nurse and coordinate with DSPs to promote independence and community inclusion.

The role requires a current Maryland LPN license and at least one year of relevant experience. A valid MD driver’s license and completion of Fello and DDA trainings are also required; part-time or full-time schedules with

Qualifications

• Must hold Maryland LPN license issued by the Maryland Board of Nursing.
• At least 1 year of nursing experience providing services for people with disabilities.
• Valid Maryland driver’s license with no more than two points.
• Willing and able to complete required Fello and DDA trainings.

Responsibilities

• Provide person-centered nursing care per Nursing Care Plan and Individual Plan, supporting ADLs.
• Coordinate medical appointments and immunizations and maintain health documentation.
• Ensure compliance with DSP qualifications and required trainings; assist with crisis responses as needed.
