Job summary

Mentor Community Services, part of the Sevita family, provides community-based living and health care services. Nurses in this role will monitor health conditions, administer medications, and oversee staff to ensure quality services for individuals served.

The position involves conducting assessments, implementing ISP health plans, and providing training on safety, infection control, and medication administration. A current LPN/LVN license and a valid driver's license are required.

Qualifications

• Associate degree in nursing or LPN program graduate.
• 1 year of experience working with population served in a nursing capacity.
• Current LPN/LVN license.
• Valid driver’s license, car registration, and auto insurance.
• CPR/FA or HIV/AIDS certification and BBP training as required by state regulations.

Responsibilities

• Monitor health conditions and ensure quality services for individuals served.
• Administer medications and oversee staff who administer medications.
• Provide physician-ordered treatments and personal care services.
• Conduct assessments and implement health care services within each ISP.
• Train staff on universal precautions, infection control, and medication administration.
• Serve as liaison with medical and professional service providers.
• Provide training on self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation.

Licensed Practical Nurse/Licensed Vocational Nurse (Primarily Administrative Duties)

• Monitor the health conditions of individuals served and ensure quality services are provided.
• Administer medications and provide documentation as well as oversee and monitor staff members who administer medications.
• Provide prescribed medical treatment by physician’s orders and personal care services.
• Conduct assessments, implement health care services to be included in each Individual Service Plan (ISP), and respond to changes of condition.
• Provide training to staff members including universal precautions, infection control, and medication administration.
• Serve as liaison and advocate with other medical and professional service providers.
• Provide training for individuals served with topics including self-medication, prescribed treatments, sex education/STD prevention and smoking cessation.

Qualifications

• Associate’s Degree in Nursing or related field, or a graduate of an accredited LPN program.
• One year of experience working with population served in a nursing capacity.
• Current state LPN or LVN License.
• Current driver's license, car registration, and auto insurance.
• Additional certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training.
• Ability to effectively communicate with staff, individuals served, and families.
