Job summary

Sevita’s NeuroRestorative division seeks a Licensed Practical Nurse (LPN) to provide direct nursing care for individuals with brain injuries and other complex needs. You will travel between multiple locations, collaborating with RNs, physicians, caregivers, and other professionals to implement individualized care plans and ensure safe, effective treatments.

The role emphasizes hands-on nursing, medication administration, and coordination with teams to support recovery, development, and community

Qualifications

  • Associate degree in Nursing or graduation from an accredited LPN program.
  • At least one year of nursing experience working with the population served.
  • Current state LPN license and any additional certifications such as CPR/First Aid, HIV/AIDS, BBP, ACT training.
  • Current driver’s license, vehicle registration, and auto insurance if providing transportation.
  • Strong communication skills and the ability to build positive relationships.
  • Self-motivated, organized, and detail-oriented with the ability to manage multiple priorities.

  • Complete health assessments and communicate findings to the RN or physician to support individualized nursing care plans.
  • Follow up on physician orders and coordinate care related to medical and dental appointments.

  • Review medication errors, identify trends, and recommend improvements to medication procedures and practices.
  • Collaborate with external healthcare providers and agencies to coordinate services and promote quality care.

Do you want a nursing role where your clinical skills directly improve the lives of others? As a Licensed Practical Nurse (LPN), you'll provide hands-on nursing care and support to individuals with developmental disabilities, acquired brain injuries, substance use challenges, and emotional or behavioral needs.

This is a dynamic role where no two days are exactly alike. You'll travel between multiple locations, collaborate closely with nurses, physicians, caregivers, and other professionals, and serve as a trusted clinical resource for individuals and the teams who support them.

What You’ll Do
  • Provide prescribed nursing care, medical treatment, and personal care services to individuals served.
  • Complete health assessments and communicate findings to the RN or physician to support individualized nursing care plans.
  • Follow up on physician orders and coordinate care related to medical and dental appointments.
  • Administer, monitor, and document medications accurately and promptly while serving as a liaison with pharmacies.
  • Review medication errors, identify trends, and recommend improvements to medication procedures and practices.
  • Conduct quality assurance reviews of healthcare records to ensure accuracy, compliance, and continuity of care.
  • Protect confidentiality, uphold individual rights, and follow universal precautions, infection-control practices, company policies, and applicable regulations.
  • Collaborate with external healthcare providers and agencies to coordinate services and promote consistent, quality care.
  • Administer and maintain required health testing and documentation for staff and individuals served.
  • Train staff on important healthcare topics, including medication administration, universal precautions, and infection control.
  • Educate individuals served on topics such as self-medication, prescribed treatments, STD prevention, and smoking cessation.
  • When needed, assist with household activities such as light housekeeping and meal preparation and provide transportation to activities and appointments.
  • Participate in the on-call rotation and provide after-hours emergency coverage as needed.
What You’ll Bring
  • Associate degree in Nursing or a related field OR graduation from an accredited LPN program.
  • At least one year of nursing experience working with the population served.
  • Current state LPN license and any additional certifications required by state regulations, such as CPR/First Aid, HIV/AIDS, Bloodborne Pathogens (BBP), and ACT training.
  • Current driver’s license, vehicle registration, and auto insurance if providing transportation to individuals served.
  • Strong communication skills and the ability to build positive relationships with individuals, families, coworkers, and healthcare professionals.
  • Self-motivated, organized, and detail-oriented with the ability to manage multiple priorities.
