Essential Job Duties and Responsibilities
Clinical Care & Health Support
  • Provide direct nursing care, including monitoring vital signs, assessing health status, and responding to medical concerns.
  • Administer medications and treatments according to physician orders and in compliance with regulatory standards.
  • Maintain accurate documentation of all nursing care, treatments, and medication administration in medical records.
  • Assist in the development and implementation of person-centered care plans.
  • Provide first aid and emergency care when needed; coordinate care with physicians and other healthcare professionals.
Individual Support & Advocacy
  • Support individuals with IDD in achieving the highest possible level of independence and quality of life.
  • Educate individuals, families, and staff on health-related topics, including hygiene, nutrition, and preventive care.
  • Advocate for the rights and dignity of each individual, ensuring all care is respectful and culturally sensitive.
Team Collaboration & Compliance
  • Work collaboratively with direct support professionals, program managers, and interdisciplinary team members.
  • Ensure compliance with state and federal regulations, facility policies, and all applicable healthcare standards.
  • Participate in staff meetings, training sessions, and ongoing professional development.
QualificationsEducation, Certifications & Experience Requirements
  • Current, valid Licensed Practical Nurse (LPN) license in Georgia
  • CPR and First Aid certification (or ability to obtain upon hire).

Work Environment & Physical Demands
  • Moderate noise level typical of office or program setting.
  • Typical physical requirements including moving personal belongings, light equipment, materials to and from meeting locations, etc.
  • Expected to be on call 24/7 unless other arrangements are made ahead of time.
  • Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions.

This is a salaried, exempt position that requires flexibility. Standard office hours are expected, but the role will require visits to community homes, attendance at evening/weekend team meetings, and participation in an on-call rotation. Work is performed in both an office environment and in community-based homes.
