Job summary

InCommunity is seeking a Licensed Practical Nurse (LPN) to deliver direct nursing care across office and community settings. You will monitor health, administer medications, document care, and coordinate with clinicians to support health plans.

The role includes health education for individuals with IDD and collaboration with the care team.

Qualifications

  • Current, valid LPN license in Georgia.
  • CPR and First Aid certification (or ability to obtain upon hire).

Responsibilities

All employees, including the License Practical Nurse (LPN) function are expected to model and promote the organization’s mission and values in every aspect of their work.Essential Job Duties and Responsibilities:Clinical Care & Health SupportProvide direct nursing care, including monitoring vital signs, assessing health status, and responding to medical concerns.Administer medications and treatments according to physician orders and in compliance with regulatory standards.Maintain accurate documentation of all nursing care, treatments, and medication administration in medical records.Assist in the development and implementation of person-centered care plans.Provide first aid and emergency care when needed; coordinate care with physicians and other healthcare professionals.Individual Support & AdvocacySupport individuals with IDD in achieving the highest possible level of independence and quality of life.Educate individuals, families, and staff on health-related topics, including hygiene, nutrition, and preventive care.Advocate for the rights and dignity of each individual, ensuring all care is respectful and culturally sensitive.Team Collaboration & ComplianceWork collaboratively with direct support professionals, program managers, and interdisciplinary team members.Ensure compliance with state and federal regulations, facility policies, and all applicable healthcare standards.Participate in staff meetings, training sessions, and ongoing professional development.Education, Certifications & Experience RequirementsCurrent, valid Licensed Practical Nurse (LPN) license in GeorgiaCPR and First Aid certification (or ability to obtain upon hire).Supervisory ResponsibilitiesNoneWork Environment & Physical DemandsModerate noise level typical of office or program setting.Typical physical requirements including moving personal belongings, light equipment, materials to and from meeting locations, etc.Expected to be on call 24/7 unless other arrangements are made ahead of time.Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions.This is a salaried, exempt position that requires flexibility.
