Job summary

UChicago Medicine is seeking a Supervisor, Cancer Community Health Workers in our Community Health department to lead a team across Ingalls Memorial Hospital and the UChicago Medicine Comprehensive Cancer Center in Hyde Park.

This role provides intensive supervision, coaching, and performance feedback, develops CHW training tools, tracks activities, and reports progress to the Manager of Community Benefit.

Qualifications

• High School Diploma or Equivalent; Bachelor's Degree preferred.
• Trained CHW with 5+ years of experience as a CHW.
• Must have a valid Driver's License and own a car.
• Familiarity with Chicago's South Side communities.

Responsibilities

• Provide intensive and ongoing supervision to the CHW team and evaluate CHWs' skills.
• Conduct 1:1 meetings, coaching sessions, and provide regular feedback to CHWs.
• Track patient/community activities, report progress, and support program quality improvement.
