Job summary

Penn Medicine is seeking a Social Work Lead to provide day-to-day operational oversight and supervision of Social Work staff within Care Coordination. The role ensures high-quality practice, adherence to policies, and timely intervention in complex psychosocial and discharge planning needs.

Responsibilities include staffing coordination, performance evaluations, and serving as a resource to the interdisciplinary team, while maintaining a limited caseload when needed.

Qualifications

• Master of Arts or Science in Social Work or related field is required.
• Licensure: LCSW or LSW required.
• 4+ years of acute care hospital social work experience with complex psychosocial needs and discharge planning.

Responsibilities

• Oversees daily Social Work operations, including assignments, caseload distribution, priorities, staffing, and coverage.
• Maintains visibility on patient care units and conducts routine staff check-ins to identify high-risk cases and discharge priorities.
• Establishes performance expectations, provides supervision, coaching, mentoring, and feedback.
• Completes evaluations and coordinates with leadership on formal corrective actions when indicated.
• Ensures Social Work practice complies with policies, NASW standards, and regulatory requirements.
• Provides guidance for complex psychosocial needs and acts as a resource to the interdisciplinary team.
