Job summary

Trilogy Health Services, LLC in Indiana is seeking a Registered Nurse (RN) to provide direct, quality clinical care and to serve as a Team Leader to patient care staff within a long‑term care setting.

You will lead a team of direct care providers, ensure accurate medications and treatments, document thoroughly, educate families, and participate in care planning and patient care conferences. Weekend package offers a $7.00/hour incentive with additional shift differentials.

Qualifications

• Must have and maintain a current, valid state RN license
• Current, valid CPR certification required

Responsibilities

• Lead a team of direct care providers to ensure medications and treatments, documentation, family teaching, care planning and patient care conferencing in accordance with policies.
• Provide direct nursing care using the nursing process and ensure continuity of care from admission through discharge and return to family.
• Visit residents daily to observe and evaluate health status and notify the charge nurse/physician/DON when changes occur.
• Identify resident problems and emergencies and initiate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing medical forms, reports, evaluations, and charting as necessary.
