Job summary

Trilogy Health Services, located in Lowell, IN, seeks an RN Team Leader to provide direct clinical care and supervise patient care staff in our Cedar Creek Health Campus. You will lead a team, ensure accurate medication administration, care planning, and thorough documentation, with daily resident reviews and escalation as needed.

The role emphasizes using the nursing process across admission to discharge, plus administrative duties and family teaching.

Qualifications

• Must have and maintain a current, valid state RN license.
• Current, valid CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure medications, treatments, documentation, family teaching, care planning and patient care conferences.
• Deliver nursing care using the nursing process and ensure continuity from admission to discharge.
• Visit residents on assigned unit daily to observe status and notify nursing leadership of changes.
• Identify resident problems and emergencies and initiate lifesaving measures if physician absent.
• Perform administrative duties: completing forms, reports, evaluations, and charting.
