Job summary

Parham Healthcare & Rehabilitation Center in Henrico, VA seeks a skilled Registered Nurse Assessment Coordinator (MDS/RNAC) to ensure timely MDS completion and RAI coordination. The RNAC will lead IDT care plan meetings and ensure accurate documentation for PDPM classification.

The role requires a Virginia RN license, with opportunities to influence facility reimbursement, survey readiness, and high-quality patient care in both short- and long-term rehab settings.

Qualifications

• Virginia RN license is required.
• Experience with MDS assessments and the RAI process.
• Strong leadership to lead IDT care plan meetings.
• Knowledge of PDPM classification and CMI data.

Responsibilities

• Schedule and lead interdisciplinary team care plan meetings.
• Implement and interpret MDS assessments.
• Review records to validate MDS accuracy.
• Code MDS items to reflect resident acuity.
• Track Case Mix Index and quality measures data.
