Job summary

FreedomCare is seeking a Registered Nurse Lead for Colorado to provide clinical leadership and supervision of the Care Management team in Denver. You will ensure safe, compliant, person-centered services and oversee care plans under Colorado Medicaid requirements.

You will collaborate with Operations and Compliance to optimize workflows, mentor Care Managers, and drive quality improvements for home healthcare delivery. A BSN is required, with leadership experience in Medicaid settings preferred.

Qualifications

• Active RN license in good standing (Compact licensure preferred).
• BSN required; MSN or related master's preferred.
• Minimum 3–5 years clinical nursing experience in home- and community-based settings.
• Minimum 2+ years clinical leadership, supervisory, or program management experience.
• Minimum 1 year developing or implementing SOPs or similar policies.

Responsibilities

• Provide day-to-day leadership and clinical guidance to the Colorado Care Management team.
• Serve as the SME for Colorado operations and support complex clinical cases.
• Review and approve care plans reflecting participant needs and regulatory requirements.
• Monitor compliance with Colorado Medicaid regulations and FreedomCare policies.
• Develop and improve clinical workflows and SOPs; support new programs and initiatives.
• Coach, supervise, and develop Care Managers; participate in recruiting and training.
