Job summary

Qualifications

• Current, valid RN license, NYS required.
• Bachelor’s degree in Nursing preferred.
• 2 years in health care with at least 1 year in home health.
• Supervisory experience preferred.
• Knowledge of Medicare guidelines governing home health.
• Strong interpersonal, organizational, time management and communication skills.

Responsibilities

• Receive calls from field staff and physicians and update care plans.
• Review and approve interim orders before sending to physicians.
• Assist Intake and process referrals in HCHB; share duties with PSC/CFSS.
• Assign Licensed Professional to conduct evaluation visits.
• Participate in case conferences and process recert/discharge decisions.
• Verify orders and follow up on missed visits and entitlements.
